Fat Injections to Improve Facial Lines- Are You a Good Candidate?

The use of fat injection has been around for a number of decades, since refinement of fat harvesting and liposuction. As the facial skin ages  the strong action of expression muscles of the face lead to visible lines in multiple areas of the face that are less than desirable. In almost all patients with advanced years in life, the amount of fat in the face is reduced and this is consistent with the aging process although it is still not clear to the medical scientists why this process occurs.  In some patients it is even more accelerated and the loss of volume from the different regions of the face gives a more pronounced aging look.

Treatment by injection of fillers has been extremely popular in the last many years. The fillers available at this time are many and they have many different characteristics but the most common prevailing characteristic is the fact that all fillers are temporary in nature and last anywhere from 3-9 months depending on the type of filler and the location of injection. Experience has shown us that injection around the lip tends to last fewer months than other parts of the face and we believe that this is due to constant activity of the lips which leads to gradual loss of the filler injected into the skin.

The injection of fat provides two distinct advantages to a select group of patients. If a patient needs multiple injections of synthetic fillers, the cost escalates rapidly as each injection adds to the cost. Fat injection, on the other hand, uses patient’s own fat and the cost of fat injection is mostly related to the time needed to remove the fat by liposuction. If a patient requires one or two syringes of Radiesse®or Juvederm Plus ®it is easily accomplished and the cost can be controlled. However, if the patient requires multiple syringes of injection, then in most cases harvesting of the fat becomes justifiable. It takes about 45min to harvest adequate amount of healthy fat usable for injection in the facial lines. At that point, the cost of fat injection becomes less than using 3, 4 or more syringes of the synthetic material to de-emphasize the facial lines of expression. An added advantage to the fat injection is that it lasts nine months to a year in most areas of the face except again around the lips where the constant movement helps degrade the fat faster which causes it to last fewer months.

Patients requesting a fat injection to the facial lines need to understand that the fat is injected underneath the line and therefore, to have an effect on the line, larger volume is needed, which adds to the contour of the face. Injection of synthetic fillers on the other hand is done into the skin substance itself and not underneath the skin and therefore volumizing requires more syringes of synthetic fillers to accomplish the same result.

Lower Eyelid Lift vs. Mid-Face Lift: What's the Difference?

Surgery to rejuvenate the face has been customized in recent years depending on different parts of the face which have different needs. Customized facial surgery allows the plastic surgeons to rejuvenate certain parts of the face without operating on other areas that still do not need any intervention yet.

Surgery of the lower eyelid to rejuvenate the area has been performed for many decades. It is usually performed to improve fat herniation and/or skin laxity from the eyelid lashes down to about 1 ½ “. On the other hand, mid-face lift is performed to improve the area from the eyelashes down to the upper lip. The improvement to be expected with the mid-face lift is much more significant than the lower lid lift.

Lower eyelid lift or blepharoplasty, as it is known, is rarely indicated alone without mid-face lifting or other additional procedures to improve the contour of the lower lid and smoothen the transition from the wrinkles underneath the eyelid to the upper lip. The mid-face lift on the other hand, is different from the traditional facelift where the stitches are only underneath the eyelashes and there are no stitches in front of the ear or behind the ear.

Both procedures are usually performed on outpatient basis and patient experiences no pain during or after the surgery. The recovery from the lower lid lift is usually 2-3 days and the mid-face lift, as it is more involved, may require a total of 4-5 days of recovery. Surgery around the eyelids is usually very well tolerated by most patients and with wearing large enough sunglasses, patient can be around people 1-2 days after surgery.
